A Swiss study of 1,745 healthcare workers found those who received a COVID-19 booster were up to 70% more likely to develop flu-like symptoms and miss work than those who didn’t get the shot. The effect was most pronounced immediately after vaccination.
The study, published Aug. 9 in Communications Medicine, a Nature Portfolio journal, tracked participants from November 2023 to May 2024 to see how vaccination status influenced respiratory illness and sick leave. The authors defined “influenza-like respiratory illness” as fever plus at least one symptom such as cough, sore throat, runny nose, or loss of smell with an acute onset in the previous week.
The researchers concluded that COVID-19 boosters “do not contribute to the protection of the healthcare workforce in a post-pandemic setting” and “may even temporarily increase the likelihood of symptomatic infection and workday loss.” They noted that their findings don’t prove causation and called for more research to confirm results and explore underlying mechanisms.
The study also found that receiving a seasonal flu shot reduced the risk of developing flu-like symptoms and missing work. That result contradicted an April preprint from the Cleveland Clinic, which reported a 27% higher flu risk among employees who received the 2024–2025 flu vaccine.
As of 2025, 10 states — California, Colorado, Connecticut, Illinois, Maine, Massachusetts, Nevada, New Mexico, New York, and Rhode Island — still require COVID-19 shots for healthcare workers. The FDA in May limited approvals for new COVID-19 vaccines to people with underlying health conditions and adults over 65. However, the CDC continues to recommend that all adults under 65 receive at least one dose of the 2024–2025 booster.
President Donald Trump issued an executive order earlier this year ending federal funding for schools that mandate COVID-19 shots, though some health sciences programs continue to require them due to loopholes.
